Comprehensive rehabilitative care focused on safely returning patients
to their homes after a hospital stay. Services include: Therapy available
up to seven days a week; post-orthopedic surgery care; physical, occu-
pational and speech therapy; pain management; wound care; advanced
intravenous therapy; nutrition management; head or spinal cord injury

The center is an outpatient memory program, helping clients and their
families manage memory impairments beyond diagnosis. All services
are customized to meet the unique needs of each client and the care team
partners with the family to adjust interventions over time. Intensive care
